How This Process Works
You will complete this form and submit it to HR. HR will review your goals against the SMART criteria and provide written feedback. Your manager will then receive both your original submission and HR's critique, and will schedule a meeting with you to finalize your goals together. All parties will sign the completed document.
These goals directly inform your quarterly check-ins, your performance review, and - for non-sales roles - your merit increase and bonus determination in April/May.

State your goal in one clear sentence. What exactly will you achieve?
List the specific numbers, KPIs, or deliverables that will confirm you have succeeded.
Break this goal into 3–5 concrete actions you will take. This proves the goal is realistic and you have a plan.
Explain how this goal connects to your team's objectives and Koshee's business priorities.
Provide a specific date - not a quarter or a vague timeframe.
List any tools, training, support from other teams, or resources required to achieve this goal.

Quarterly Check-in Dates
Your manager will meet with you at the start and end of each quarter to review progress

By signing below, both the employee and manager confirm that these goals have been reviewed, discussed, and mutually agreed upon. Both parties commit to the quarterly check-in schedule and to providing honest, timely feedback throughout the review period.
